The City of Boroondara's Active Ageing Services provide low level aged care services through the Commonwealth Home Support Programme (CHSP).
If My Aged Care assesses you as having more complex needs, you may be eligible for:
A Home Care Package (HCP), which is an allocation of money that you can spend on services of your choice. Package services are purchased from private providers. My Aged Care will help you to find an appropriate package provider for you.
Allied health and therapy services, such as nursing, podiatry, physiotherapy and transition care.
Aged care residential homes which offer different levels of service for different health and lifestyle needs including fully supported accommodation, personal care and health care.
The City of Boroondara does not provide any of these services.
