Wellbeing Conversation: Living well at home with a terminal illness

Are you living with a terminal illness, caring for someone who is, or just wanting to plan ahead? Join us online with a representative from Eastern Palliative Care (EPC) who will provide you with the information you need to feel more informed, supported and confident as you navigate this stage of life.

Topics covered will include:

• Advance Care Planning
• how you can live well at home, or support someone to live well with a terminal illness
• what Community Palliative Care is
• EPC specialist services including nursing, family support and allied health, and who is eligible for their services
• volunteer services such as biography writing, dog walking, photography and respite companions.

This program is part of the Boroondara Wellbeing Conversation Series and is being delivered in partnership with City of Monash and City of Maroondah.
