Assessment and care management
To receive services you will need to be assessed for eligibility and to make sure suitable services are delivered.

To find out if you, or someone you know, can receive services, contact the Ageing and Disability Services Intake Office - open 8.30am to 5.15pm Monday to Friday (excluding public holidays).
The person needing services must give their consent. The Intake Officer will collect information and check eligibility for service.

Assessment
An assessment officer will organise a time to visit you in your home to find out what help you may need. They will discuss your strengths, needs and existing supports.
Assessment officers can link you into other local services within and outside Council, including social programs and health promotion activities.
Reviews
Reviews help us identify and respond to any changing needs you may have. We aim to review all our clients every year.
We can also review your services upon request, or if we get feedback your needs have changed.
Fees for services
While services are subsidised by the Department of Human Services (DHS), fees do apply and are based on an assessment of your income and ability to pay.
They are also based on the type of service you need. DHS provides guidelines on fees and charges.
